Customer Service:Your First Visit
We understand that choosing a dental provider is based on good chemistry so please feel free to ask any questions that you may have. We'll spend the time so you can understand the answers. If you have decided to schedule a visit with us, there are a few things you should know about your first visit.
What to Bring
We will have forms for you to fill out. These forms gather information about your medical and family history as well as information about your dental plan. Make sure you disclose all medical problems including any allergies you may have and all medications you are taking.
Also bring the following:
- INSURANCE CARD - If you have insurance, bring the insurance card if available.
- DRIVER'S LICENSE - We will need a copy of your drivers license to verify it is you that will be utilizing your insurance benefits if aplicable.
- PAST DENTAL X-RAYS - Any dental x-rays taken within the last six months.
- RESPONSIBLE PARTY - The person responsible for payment on the account will need to be present to sign the paperwork. This will not be necessary for future appointments.
What to Expect
Below are the steps to ensure a successful first visit:
- Fill out paperwork to complete so we may get to know you as well as get an understanding of your needs.
- We will make copies of the need documents stated above.
- The Doctor's dental assistant will greet you in the reception area.
- The dental assistant will discuss your needs and document appropriately.
- Necessary x-rays are taken in order for the Doctor to be able to see the teeth.
- Once the x-rays are developed, the Doctor will come in to meet you.
- You and your Doctor can discuss your needs and options.
- The dental assistant will present you with a treatment plan so that you will have an estimated cost if any.
- You will be given the opportunity to discuss any further questions with your Doctor.
- Our patient coordinators will help you schedule all recommended appointments.

Our goal is to help you keep your teeth for a lifetime so you can feel better about yourself and your smile. Our treatment recommendation will be based on your individual needs. We'll discuss options with you and together we can figure out a game plan to keeping a lifetime of healthy and great looking teeth.
American Dental Association Codes
Listed below are some of the most common codes that may be billed on your first visit. You may contact your insurance company in advance to inquire about the initial cost.
- Comprehensive Exam D0150 - A complete exam but not when coming in for a specific problem in one area.
- Limited Exam D0140 - Coming in for a specific problem area.
- Bitewing X-rays D0274 - X-rays may be needed in conjunction with either exams listed above.
- Panoramic X-ray D0330 - X-ray may be needed in conjunction with either exams listed above.
- Periapical X-rays D0220 - X-rays may be needed in conjunction with either exams listed above.
